At Eyevista, your eye health is our primary concern. Our provider, Dr. Jack Chan attends frequent meetings and education courses to learn about advances in lens design, materials, and coatings.
His trained staff are committed to personalized care for the patients of this community. We strive to give you a memorable customer experience, offering a thorough and attentive eye examination and assisting in frame selection in a friendly environment. Our office offers a full range of vision services and carries quality fashion eyewear.
Dr. Jack Chan studied at Columbia University and SUNY College of Optometry with a Doctorate of Optometry and also a Masters in Vision Science.
With continuing education, he strives to provide superior eyecare with the latest information. Our office is constantly adding new technology and instrumentation to stay at the cutting-edge of our profession.
During his free time, Dr. Chan enjoys traveling, dining out, and reading books. On the weekends, he also spends time with his family finding new places to explore in Brooklyn and attending church and helping out.
Dr. Chan currently resides in Bensonhurst, Brooklyn, and is proud to practice in the same community.

Dr. Jack Chan
Dr. Janice Tan
Dr. Janice Tan is a board-certified optometrist. She grew up in Brooklyn and earned her Doctor of Optometry degree from Pennsylvania College of Optometry. She completed her clinical training at Seidenberg Protzko Eye Associates in Maryland, Brooklyn VA Hospital, and Broward Eye Care in Florida.
Dr. Tan provides comprehensive eye care and has a special interest in myopia control and dry eye management. She is a great communicator with patients and cares for her patients above and beyond.
Dr. Tan is fluent in Mandarin and Cantonese.
